What it is, How it works

Hair examination has increasingly become recognized as an effective method for identifying drug and alcohol consumption. Hair retains a long-term record of alcohol and drug use by capturing biomarkers within the hair shaft as it elongates. When collected near the scalp, hair can reveal up to about a 90-day detection period for alcohol and substances. Hair collection is straightforward, somewhat resistant to tampering, and convenient for transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for analysis and validation. For EtG, additional tests, or panels exceeding 10, a 150mg specimen is advisable. Employ a jeweler’s scale for accuracy in weighing the sample. If scalp hair is absent, a similar quantity of body hair may be obtained. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The drug test process in laboratories involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory phase of logging a sample into a laboratory’s system. This step involves ensuring the sample was securely sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and inputting any supplementary data not available through an electronic chain of custody platform.

Screening includes a preliminary check for drug substances. Although Screening is a cost-effective means to dismiss drug presence in most samples, a positive screen necessitates confirmation for use in legal proceedings. Samples which test presumptively positive in Screening must undergo additional confirmation.

If Screening results are presumptive positive, extra hair is extracted from the original sample for Extraction. Here, drugs are retrieved from hair at much lesser concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing highly complex.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation. The full laboratory chain from Accessioning to Confirmation is assessed under both CAP (College of American Pathologists) Hair designations and in accordance with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
