A 1.5-inch section of around 200 hair strands (about the diameter of a #2 pencil) near the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. It is advisable to weigh the specimen with a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be gathered. Reference to head hair indicates scalp hair only, while body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedure for a drug test result comprises four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where the sample is processed into the laboratory’s system. This step includes confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening entails a preliminary quick check for drugs of abuse. Although Screening is a cost-effective approach to rule out drug usage in most samples, a positive screen must be confirmed for it to be admissible in court. All samples that show a presumptive positive in Screening require secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, more hair is drawn from the initial specimen for the Extraction stage. During this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to perform.

Confirmation of any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
