Hair is effortlessly gathered, relatively hard to adulterate, and straightforward to transport.

A 1.5-inch section consisting of around 200 hair strands (approximating the thickness of a #2 pencil) from nearest the scalp provides 100mg of hair, which is optimal for screening and confirmation purposes. For EtG, additional, and/or tests beyond 10 panels, 150mg of the sample is suggested. It's advisable to measure the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be taken. By "head hair", we mean solely scalp hair. Body hair encompasses all other hair types (like facial, axillary, etc.).

Process Overview

There are four primary steps in laboratory processing for drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into the lab’s system. It involves checking that the sample was properly sealed and shipped, attaching a random LAN (Laboratory Accessioning Number), and doing any extra data entry not supplied by an electronic chain of custody system.

Screening means a quick preliminary scan for drugs of abuse. Although Screening is a cost-efficient method for eliminating drug consumption in most samples, a positive result requires confirmation to stand up in court. Any sample that shows a presumptive positive result in Screening mandates a secondary confirmation.

If a sample yields a presumptive positive during Screening, additional hair is extracted from the original specimen for further processing in Extraction. At this point, drugs are extracted from hair at significantly lower concentrations than alternative methods (like urine or oral fluid), making hair drug screening the most complex method to execute.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumptively positive undergo washing prior to confirmation where necessary. The entire laboratory operation from Accessioning to Confirmation is audited according to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

    The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
