What it is, How it works

Hair captures a prolonged history of alcohol and drug use by embedding biomarkers within the fibers of the developing hair strand. When obtained near the scalp, hair can offer up to a roughly 3-month window for detecting alcohol and drug consumption. Hair is straightforward to gather, somewhat challenging to tamper with, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (around the width of a #2 pencil) nearest the scalp yields 100mg of hair, ideal for both screening and confirmation. For EtG, additional tests, and/or panels beyond 10, 150mg of the sample is advised. It is recommended to measure the specimen using a jeweler’s scale. If scalp hair isn't available, a comparable amount of body hair can be gathered. References to head hair pertain to scalp hair exclusively. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial step of integrating a sample into a laboratory’s system. This step ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finalizes any supplementary data entry not covered by an electronic chain of custody system.

Screening includes a preliminary swift evaluation for drugs of abuse. While Screening is a cost-efficient strategy to negate drug consumption for most samples, a positive result requires confirmation to be valid in court. Any samples that appear positive in Screening require subsequent confirmation.

Should a sample appear positive during the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. During this phase, drug extraction from hair occurs at significantly reduced concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is deemed the most challenging method to execute.

Confirmation of any positive Screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are tentatively positive undergo washing prior to confirmation as necessary. The entire laboratory workflow from Accessioning to Confirmation undergoes review according to both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
