What it is, How it works

Hair tests have gained recognition as a significant method for detecting drug and alcohol consumption. Hair strands harbor long-term insights into alcohol and other substance usage by capturing biomarkers within the growing hair's fibers. When gathered at the scalp, hair can reveal up to a 3-month time frame for detecting alcohol and various drugs. Hair is easy to gather, relatively hard to tamper with, and convenient to send.

A 1.5-inch piece of roughly 200 hair strands (around the width of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample for testing and verifying. For EtG, additional tests, and/or panels exceeding 10, 150mg of the hair sample is suggested. It's advisable to measure the specimen using a jeweler's scale. If scalp hair isn't accessible, an equivalent quantity of body hair can be collected. "Head hair" pertains solely to scalp hair. "Body hair" includes all other hair kinds (facial, axillary, etc.).

Process Overview

The main four steps in laboratory analysis of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step of processing a sample within a lab's system, which includes checking the sample's proper sealing and shipping, allocating a random LAN (Laboratory Accessioning Number), and finishing any further data input not handled by an electronic chain of custody system.

Screening is an initial swift check for drugs of abuse. While Screening serves as an economical means to exclude drug use for the majority of samples, positive results must be confirmed to be acceptable in legal proceedings. Samples that are tentatively positive in Screening necessitate a secondary confirmation.

If a sample shows tentative positivity during Screening, additional hair is extracted from the initial sample for Extraction. During Extraction, drugs are drawn from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug screening a challenging procedure to execute.

Any positive Screen results are confirmed through processes like GC/MS, GC/MS/MS or LC/MS/MS. All samples with potential positivity undergo washing prior to confirmation as required. The comprehensive lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
