When collected near the scalp, hair can offer up to an approximately 3-month detection period for alcohol and other substances. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch piece of about 200 hair strands (comparable in size to a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional tests, or panels exceeding 10, 150mg of the sample is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is not available, an equivalent quantity of body hair can be used. When discussing head hair, we are specifically referring to scalp hair. Body hair denotes any other types of hair (facial, axillary, etc.).

Process Overview

The four essential steps in the lab processing of a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a laboratory’s framework. This process includes confirming that the sample was securely sealed and dispatched correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing additional data entry not furnished by an electronic chain of custody system.

Screening consists of a preliminary rapid check for substances of abuse. While Screening is a cost-efficient approach to rule out drug usage in most samples, a positive screen requires confirmation for admissibility in legal proceedings. Any samples that test presumptively positive in Screening require a secondary confirmation.

If a sample tests presumptively positive in the Screening stage, additional hair is extracted from the initial sample for Extraction. During this phase, substances are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Confirmation of any positive screen result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary.
