What it is, How it works

Hair analysis is now recognized as an effective method for identifying drug and alcohol consumption. When hair is cut near the scalp, it can reveal drug and alcohol use over a period of roughly three months. Hair sampling is straightforward, not easily tampered with, and can be transported with ease.

A 1.5-inch portion of approximately 200 hair strands (comparable to a #2 pencil) taken close to the scalp will yield 100mg, which is ideal for both screening and verifying results. For EtG screenings, additional tests, or panels exceeding 10, a sample of 150mg is preferable. It's advisable to measure the sample with a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair can be collected. 'Head hair' refers exclusively to scalp hair, while 'body hair' includes all other hair types (facial, axillary, etc.).

Process Overview

There are four key phases in the lab processing for drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is logged into the laboratory’s system. This step ensures the sample is correctly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and finalizes additional data input not available from the electronic chain of custody system.

Screening is an initial assessment for drug presence. While Screening is an economical means to exclude drug use in most samples, a positive finding necessitates confirmation to be legally binding. Samples rendering a positive result in Screening must undergo secondary confirmation.

Should a sample test positive during Screening, additional hair is extracted from the original sample and prepped for Extraction. In this stage, drugs are extracted from hair at lower concentrations compared to methods like urine or oral fluids, making hair drug screening one of the more challenging methodologies to execute.

Confirmation of a positive Screening result is achieved through techniques such as GC/MS, GC/MS/MS or LC/MS/MS. Any presumptively positive samples are cleansed before confirmation as required. The lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair protocol and the accreditation to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
