For EtG, additional tests, or those exceeding 10 panels, 150mg is recommended. We suggest utilizing a jeweler’s scale for accuracy. If scalp hair is not accessible, a comparable amount of body hair is acceptable. References to head hair indicate scalp hair exclusively. Body hair encompasses other types (e.g., facial, axillary).

Process Overview

Four key stages comprise the laboratory processing of drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab’s framework. This comprises ensuring the sample's integrity during shipment, assigning a unique LAN (Laboratory Accessioning Number), and inputting any supplementary data that an electronic chain of custody system omits.

Screening is a rapid preliminary check for drug abuse. While cost-effective for negating drug presence in most samples, a positive result requires confirmation for legal acceptance. Samples yielding a positive indication in Screening necessitate further verification.

If Screening suggests the presence of drugs, additional hair is extracted from the sample for the Extraction phase. Here, substances are isolated from hair at significantly lower concentrations than in other methods (such as urine or saliva), highlighting the complexity of hair drug analysis.

Confirmation follows for any positive Screening result using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All positive samples are cleansed prior to Confirmation if needed. The complete lab process, from Accessioning to Confirmation, undergoes scrutiny under CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
