For the optimal screening and confirmation, a 1.5-inch sample comprising roughly 200 hair strands (akin to the thickness of a #2 pencil) close to the scalp yields 100mg, which is the preferred sample weight. For tests involving EtG add-ons or extending beyond 10 panels, 150mg is advised. For accuracy, weigh the sample on a jeweler’s scale. If scalp hair is not accessible, a corresponding amount of body hair can be used. By head hair, we mean only scalp hair; body hair encompasses all other hair types including facial and axillary.

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes basic integration into the laboratory’s system. This step ensures the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and concludes any remaining data entry absent from an electronic chain of custody system.

Screening involves an initial, swift survey for substance misuse. Screening serves as a cost-effective approach to eliminate the usage of drugs in the majority of cases; however, a positive screen requires confirmation to stand valid in court. Samples that test presumptively positive must undergo additional verification.

If a sample shows presumptively positive during Screening, further hair is retrieved from the initial sample for Extraction. Drugs are isolated from hair at a notably lower concentration compared to other methods (such as urine or oral fluid), presenting a greater challenge than other drug screening methodologies.

Verification of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples receive a wash before confirmation as required. The entire laboratory workflow, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
