Employers with strict no-drug policies, as well as courts and probation officers, prefer hair follicle testing for its extended detection range of up to 90 days, rigorous collection methods, and difficulty to evade, although urine tests remain superior for detecting recent use within the past 5 days.

What it is, How it works

Hair testing is perceived as an effective method for detecting drug and alcohol use. Hair captures a long-term history of alcohol and other substances by ensnaring biomarkers in the strands of the growing hair. Collected near the scalp, hair offers roughly a 3-month window for detecting alcohol and other drugs. Hair is easy to gather, challenging to adulterate, and simple to send.

A 1.5-inch segment of about 200 hair strands (around the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal sample for initial screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the sample is advised. It's prudent to weigh the sample on a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair can be collected. When referencing head hair, we mean scalp hair only. Body hair describes all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a sample into a lab’s system. This process checks that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic chain of custody system.

Screening offers an initial swift inspection for drugs of abuse. While Screening is cost-effective for ruling out drug use in most samples, a positive screen requires confirmation to be admissible in court. Samples that test positive in Screening necessitate secondary confirmation.

If a sample tests positive in the Screening stage, more hair is drawn from the original specimen for Extraction. In this stage, drugs are extracted from hair at a far lower concentration than other methodologies (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to undertake.

Any positive screening result undergoes Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation as needed. The entirety of the laboratory process from Accessioning to Confirmation is reviewed under CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
