What it is, How it works

The collection process is straightforward, and tampering with the sample is challenging, plus the samples are easily transportable.

A sample consisting of a 1.5-inch section of roughly 200 hair strands (approximately the width of a #2 pencil) from the scalp equates to 100mg, which is the optimal amount for both screening and validation. For testing involving EtG, additional panels, or exceeding 10 panels, 150mg of hair should be gathered. It is advisable to measure the sample using a jeweler’s scale. In cases where scalp hair cannot be obtained, a comparable quantity of body hair is acceptable. "Head hair" denotes scalp hair exclusively, whereas "body hair" encompasses all other varieties (face, underarms, etc.).

Process Overview

There are four key phases in the lab processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where the sample is integrated into the lab’s system. This ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry not handled by an electronic chain of custody system.

Screening involves a preliminary assessment for drugs of abuse. It is a cost-efficient method to exclude drug use in the majority of samples, but a positive result requires confirmation to be suitable as legal evidence. Any samples that test as tentatively positive in Screening necessitate further verification.

If a Screening result is tentatively positive, additional hair is extracted from the original sample for the Extraction phase. Drugs are separated from hair at significantly lower concentrations compared to other methods (e.g., urine or saliva), making hair drug screening the most intricate to execute.

Confirmation of any positive result from the Screening is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All tentatively positive samples receive a wash prior to confirmation when necessary. The full lab process from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
