A hair sample of 1.5 inches consisting of about 200 strands (comparable in size to a #2 pencil) near the scalp provides 100mg of hair, which is the perfect amount for both screening and confirmation. For EtG, add-ons, and tests beyond 10 panels, 150mg of the specimen is advisable. Utilization of a jeweler’s scale for weighing the sample is suggested. In cases where scalp hair is not accessible, an equal quantity of body hair is acceptable. When discussing head hair, it specifically pertains to scalp hair. Body hair encompasses all other types, such as facial and axillary hair.

Process Overview

The laboratory process for drug test results comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the primary processing of a sample within the lab system. This phase includes validation of proper sealing and shipping of the sample, allocation of a random LAN (Laboratory Accessioning Number), and any supplementary data entry absent from an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of concern. Although Screening is an economical method to eliminate drug usage in most samples, a positive result requires confirmation for legal admissibility. Any samples potentially positive in Screening need confirmatory analysis.

Should a sample indicate a positive result during Screening, additional hair is extracted from the initial collection for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug analysis the most complex technique to execute.

Confirmation of any positive screened result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positives undergo washing before confirmation, if necessary. The entire lab process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair standards and accredited to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
