A collected sample of 1.5 inches, consisting of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp, provides 100mg of hair, the perfect quantity for both screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, a sample of 150mg is advised. It is suggested to weigh the sample with a jeweler’s scale. In cases without scalp hair, an equivalent amount of body hair can be taken. When mentioning head hair, it is solely in reference to scalp hair. Body hair pertains to all other types (such as facial, axillary, etc.).

The main laboratory steps for processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially introduced into a laboratory's system. This step ensures that the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and performs any additional data entry overlooked by an electronic chain of custody system.

Screening executes a preliminary straightforward check for drugs of abuse. Although Screening provides a cost-efficient method to dismiss drug use for the majority of samples, a positive screen needs confirmation for court acceptance. Any samples that test presumptively positive in Screening require a secondary confirmation.

When a sample is detected as presumptively positive during the Screening stage, more hair is removed from the original specimen and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening a challenging methodology to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples undergo washing as necessary before confirmation. From Accessioning to Confirmation, the entire laboratory procedure undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.
