What it is, How it works

A 1.5-inch sample consisting of roughly 200 hair strands (about the girth of a #2 pencil) taken adjacent to the scalp will yield 100mg of hair, which is the optimal quantity for initial and confirmatory testing. For EtG, additional panels, or tests exceeding 10 panels, a recommendation of 150mg of specimen is suggested. It's advised to measure the specimen with a jeweler’s scale. If scalp hair is unavailable, an equivalent volume of body hair may be harvested. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the laboratory’s system. This step involves checking that the sample was adequately sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data tasks not included in an electronic chain of custody system.

Screening is an initial rapid inspection for drugs of abuse. Although Screening is an economical approach to rule out drug use on most samples, a positive screen must be confirmed for court admissibility. Samples that are presumptively positive in Screening require secondary verification.

For any presumptively positive sample in the Screening phase, additional hair is obtained from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Any positive screening outcomes are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
