When collected near the scalp, hair offers up to a roughly 3-month detection period for alcohol and other substances. Hair is easy to gather, relatively hard to alter, and simple to send.

A 1.5-inch sample comprising about 200 hair strands (similar to the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, the perfect amount for both screening and confirmation. For EtG, extensions, and/or examinations exceeding 10 panels, 150mg of the material is advised. Utilizing a jeweler’s scale for weighing is recommended. If scalp hair isn't available, a comparable volume of body hair can be obtained. By head hair, we specifically mean scalp hair. Body hair refers to all other hair categories (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four critical steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab’s system. This step verifies that the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not included in an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Though Screening effectively rules out drug use for most samples at a lower cost, a positive result requires confirmation for court admissibility. Samples that are presumably positive during Screening must undergo secondary confirmation.

If a sample is presumed positive during the Screening phase, further hair is extracted from the initial sample and prepared for Extraction. At this juncture, drugs are extracted from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is among the most challenging methodologies to execute.

Confirmation of a positive screening result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation, as necessary. The comprehensive laboratory process from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
