What it is, How it works

Hair analysis has emerged as an effective method for uncovering drug and alcohol consumption. The fibers of the growing hair strand capture biomarkers, offering a prolonged record of alcohol and drugs. When hair is taken near the scalp, it can reveal substance use for up to three months. Collecting hair is straightforward, challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches and consisting of around 200 hairs (similar in size to a #2 pencil) closest to the scalp provides 100mg of hair, which is optimal for screening and verification. For EtG, enhancements, and tests exceeding 10 panels, 150mg of hair is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't available, an equivalent amount of body hair can be gathered. Head hair pertains solely to scalp hair, while body hair encompasses other kinds (facial, axillary, etc.).

Process Overview

The key stages in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a laboratory's system. This step includes ensuring the sample was properly sealed and sent, issuing a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient method for excluding drug use in most samples, a positive screen must be verified to be court-admissible. Presumptive positive samples in Screening require a subsequent confirmation.

If a sample indicates a presumptive positive in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or saliva), making hair drug screening the most intricate method to execute.

Confirmation of any presumptive positive screening result is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation as needed. The entire lab process from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
