For EtG, additional add-ons, and tests surpassing 10 panels, a 150mg specimen is advised. We suggest using a jeweler's scale for accurate specimen weight. If scalp hair is unavailable, an equivalent quantity of body hair may be used. By "head hair," we mean only scalp hair; "body hair" includes all other types like facial or axillary hair.

Process Overview

The laboratory process for drug test results involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of processing a sample within a laboratory system. This phase ensures the sample is sealed and shipped correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not handled by an electronic chain of custody system.

Screening is a preliminary, rapid evaluation for drugs of abuse. Although Screening effectively eliminates drug use in most samples affordably, any positive result requires Confirmation to be court-admissible. Thus, presumptive positive samples in Screening undergo a secondary confirmation.

If Screening indicates a presumptive positive, more hair is extracted from the original specimen for the Extraction phase. Here, drugs are extracted at much lower concentrations than other methods such as urine or oral fluids, making hair drug testing one of the most complex methodologies.

GC/MS, GC/MS/MS, or LC/MS/MS is used to confirm any positive screening outcomes. All samples with presumptive positives are washed before confirmation if necessary. The process from Accessioning to Confirmation is thoroughly examined under CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
