What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol usage. Hair serves as a long-term record of alcohol and other drug usage by encapsulating biomarkers in the strands of growing hair. When obtained close to the scalp, hair offers up to roughly a 3-month time frame for detecting alcohol and drugs. Hair is easy to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch section consisting of around 200 strands of hair (approximately the size of a #2 pencil) near the scalp provides 100mg of hair, which is the preferred sample size for screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the sample is advisable. We recommend using a jeweler’s scale to measure the specimen. If scalp hair cannot be obtained, a comparable amount of body hair may be gathered. In discussing head hair, we specifically mean scalp hair. Body hair includes all other hair types (such as facial, axillary, etc.).

Process Overview

The key steps in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory's system. This involves checking that the sample was securely sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening is an initial rapid test for drugs of abuse. Although Screening is an economical means to rule out drug use in most samples, a positive screen must be confirmed to be legally acceptable. Samples that test presumptively positive in Screening must undergo a secondary confirmation.

If a sample is found to be presumptively positive in Screening, additional hair is taken from the initial sample for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

The confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive are washed before confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized both under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
