For EtG and other additional tests exceeding ten panels, a specimen of 150mg is preferred. Using a jeweler’s scale is advisable for specimen weighing. In the absence of scalp hair, a similar quantity of body hair can be used. "Head hair" specifically pertains to scalp hair, whereas "body hair" covers other types like facial or axillary hair.

Process Overview

The laboratory analysis process for drug testing incorporates four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's system. This step verifies correct sealing and shipment of the sample, issues a random LAN (Laboratory Accessioning Number), and completes any necessary data entry absent from an electronic chain of custody.

Screening conducts a preliminary rapid assessment for substance abuse. Although cost-efficient for excluding drug usage in most cases, a positive screen necessitates confirmation for legal acceptance. Presumed positive samples in Screening must undergo further confirmation.

In cases of a presumed positive result during Screening, additional hair is extracted from the initial sample for Extraction. This phase extracts drugs from hair at significantly lower concentrations than other methods (e.g. urine or oral fluids), accentuating the complexity of hair drug screening.

Confirmation for any positive findings from screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positives are washed as necessary. The entire laboratory procedure, from Accessioning to Confirmation, is audited according to both the CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.
