What it is, How it works

Hair examination has emerged as an influential means for identifying drug and alcohol consumption. Hair offers a prolonged historical record of alcohol and drugs by encapsulating biomarkers within the fibers of the hair strand as it grows. When sourced close to the scalp, hair can offer up to approximately a 3-month detection timeframe for alcohol and drugs. Hair is straightforward to collect, somewhat hard to adulterate, and easy to transport.

A 1.5-inch segment of about 200 hair strands (similar in thickness to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for analysis and confirmation. For EtG, additional panels, and/or tests beyond 10 panels, 150mg of the sample is preferable. Utilizing a jeweler’s scale is recommended for weighing the specimen. If scalp hair is unavailable, an equivalent amount of body hair may be utilized. By head hair, we specifically mean scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary steps involved in laboratory evaluation of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the lab's system. This includes verifying that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drug abuse. While Screening is an economical method to rule out drug use in the majority of samples, a positive screen must be validated to be admissible in a legal setting. Any samples that test presumptively positive in Screening require further confirmation.

Upon a presumptive positive result in the Screening stage, additional hair is extracted from the original sample and prepared for Extraction. During this stage, drugs are isolated from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Verification of any positive screen result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positive are washed prior to confirmation as required. The full laboratory process from Accessioning to Confirmation is reviewed in accordance with both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Advantages of hair drug testing:

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
