A 1.5-inch sample consisting of about 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample for screening and confirmation. For tests like EtG, add-ons, and/or those exceeding 10 panels, a 150mg specimen is suggested. We advise measuring the specimen using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be gathered. "Head hair" refers specifically to scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a specimen into the laboratory system. This includes ensuring that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any necessary data entry not covered by an electronic chain of custody mechanism.

Screening entails a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to rule out drug use in most samples, a positive result requires confirmation for legal admissibility. Samples that test presumptively positive in Screening necessitate a further confirmation procedure.

For samples testing presumptively positive at the Screening stage, additional hair is extracted from the original specimen for the Extraction process. During this step, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methods to perform.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples undergo washing prior to confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
