What it is, How it works

When the hair is taken near the scalp, it can offer a detection window of up to approximately 3 months for alcohol and other substances. It is straightforward to collect hair, somewhat challenging to alter, and convenient to transport.

A 1.5-inch sample of around 200 hair strands (comparable to the size of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair is not accessible, an equivalent quantity of body hair may be collected. When we mention head hair, we specifically mean scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The main steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of processing a sample into a lab’s system. This step ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not provided by an electronic chain of custody system.

Screening is a preliminary quick check for drugs of abuse. While Screening is an economical means to dismiss drug use in most samples, a positive screen requires confirmation to be court-admissible. Any samples presumed positive during Screening necessitate a secondary confirmation.

For samples presumptively positive in Screening, more hair is sourced from the initial specimen for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All potential positive samples are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
