This method is favored by courts and probation officers as well. Its major advantage is a 90-day detection window, with collection observed to ensure accuracy. Despite this, urine tests are still best for identifying recent drug use within five days.

What it is, How it works

Hair testing has become recognized as an effective method for detecting drug and alcohol consumption. By encasing biomarkers within the fibers of the growing hair strand, hair provides a long-term record of alcohol and drug usage. When sourced close to the scalp, hair can offer up to a 3-month window for detecting alcohol and other drugs. Hair is easy to collect, somewhat resistant to tampering, and simple to transport.

Providing a 1.5-inch sample of around 200 hair strands (roughly the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the specimen is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair is missing, a comparable quantity of body hair can be collected. When mentioning head hair, we refer solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the sample's initial processing into a lab’s system. It entails confirming the sample was sealed and shipped accurately, assigning a random LAN (Laboratory Accessioning Number), and completing any additional necessary data entry not provided by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. While Screening is a cost-effective method to exclude drug use on the majority of samples, a positive screen demands confirmation to be permissible in court. Any samples presumptively positive in Screening require further confirmation.

If presumptively positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than other methodologies (such as urine or oral fluid), which is why hair drug screening is a complex methodology.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The complete lab procedure from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
