A 1.5-inch section containing approximately 200 hair strands (size comparable to a #2 pencil) gives 100mg, which is the optimal amount for analysis. For tests including EtG, add-ons, or those exceeding 10 panels, 150mg is advised. It's best to measure the sample with a jeweler's scale. If scalp hair can't be used, equivalent body hair may be sampled. By head hair, we specifically mean scalp hair, whereas body hair includes all non-scalp hair (like facial or axillary hair).

Process Overview

The primary stages in lab processing of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails entering a sample into the laboratory's workflow. This step involves confirming proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any additional data needs not covered by an electronic chain of custody.

Screening is a preliminary assessment for illicit drugs. Though Screening is cost-efficient for dismissing drug use in most cases, a positive result must be confirmed for legal acceptability. Samples that show positive in Screening need confirmation through further testing.

If Screening indicates a presumptive positive, additional hair is used for Extraction. In this extraction stage, drugs are isolated from hair at much lower levels compared to methods like urine or oral fluid testing, highlighting the challenge of hair drug screening.

Confirmation for any positive Screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to Confirmation, all presumptive samples undergo necessary cleansing. The entire process from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
