What it is, How it works

Hair analysis has gained recognition as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and drug use by embedding biomarkers within the structure of the growing hair strand. When collected close to the scalp, hair can offer up to around a 3-month detection timeframe for alcohol and drugs. Hair is straightforward to collect, somewhat resistant to tampering, and easy to transport.

A 1.5-inch section comprising roughly 200 strands of hair (approximating the size of a #2 pencil) near the scalp will yield 100mg of hair, the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent volume of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The process for laboratory drug test analysis includes four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This process confirms that the sample was appropriately sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not handled by an electronic chain of custody system.

Screening involves a preliminary rapid analysis for drugs of abuse. Although Screening is an economical way to eliminate drug use in most samples, positive screenings need confirmation for legal admissibility. Samples that test presumptively positive during Screening require further confirmation.

When a sample tests presumptively positive in the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (ex. urine or oral fluid), making hair drug analysis the most challenging method to perform.

Confirmation for any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The complete laboratory workflow from Accessioning to Confirmation is thoroughly reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
