For EtG, additional tests, and any analysis beyond 10 panels, a specimen weighing 150mg is advisable. It is suggested to measure the sample weight using a jeweler's scale. If scalp hair cannot be collected, a comparable quantity of body hair can be used. References to head hair specifically mean scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

There are four core steps involved in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial process of integrating a sample into the laboratory's system. This step involves checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not already included by an electronic chain of custody system.

Screening provides an initial rapid assessment for substances of abuse. While Screening is a cost-efficient method to dismiss drug usage in most samples, a positive result in Screening must be verified to be valid in legal contexts. All samples with a presumptive positive in Screening necessitate a follow-up confirmation.

When a sample shows a presumptive positive during Screening, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are withdrawn from the hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening one of the most challenging techniques to execute.

Positive results in Screening are corroborated through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation if required. The complete lab process from Accessioning through Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
