A sample measuring 1.5 inches long, consisting of about 200 hairs (approximately the circumference of a #2 pencil) nearest the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and tests beyond 10 panels, a 150mg specimen is suggested. We advise that the sample be weighed using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair is acceptable. Head hair pertains exclusively to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The four essential steps in laboratory processing for a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of initially entering a sample into a laboratory's system. It involves verifying that the sample was properly sealed and dispatched, assigning a LAN (Laboratory Accessioning Number) at random, and completing any additional data entry not covered by an electronic chain of custody.

Screening involves an initial rapid assessment for substances of abuse. While Screening is a budget-effective method to rule out drug consumption in most samples, any positive findings need confirmation to hold up in legal proceedings. Samples that are possibly positive require further confirmation tests.

If Screening indicates potential positivity, more hair is extracted from the original sample and readied for Extraction. During Extraction, drugs are removed from hair at a far lower concentration than with other methods (e.g., urine or oral fluid), underscoring why hair drug testing is the most challenging approach to conduct.

Confirmation for any positive Screening results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that show potential positivity are washed prior to Confirmation as necessary. The whole laboratory procedure from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) standards for hair analysis and accreditation under ISO / IEC 17025 norms.
