What it is, How it works

When taken near the scalp, hair can reveal alcohol and drug usage for up to three months. It is easy to collect, relatively resistant to tampering, and convenient to ship.

A 1.5-inch segment of about 200 hair strands (approximately the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale for precise measurement. If scalp hair is not available, a comparable amount of body hair may be used. We refer to scalp hair exclusively when mentioning head hair, while body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is composed of four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of introducing a sample into the lab's system. This step includes checking that the sample was correctly sealed and transported, assigning a Lab Accessioning Number (LAN) at random, and completing any necessary data entry that an electronic chain of custody system does not cover.

Screening provides an initial rapid assessment for drugs of abuse. Although this step is cost-effective for excluding drug use in most samples, any positive results require confirmation to be valid in court. Presumptive positive findings from Screening require additional confirmation.

Should a sample indicate a presumptive positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are separated from hair at much lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening a challenging process to carry out.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo a wash before confirmation if necessary. The complete laboratory procedure, from Accessioning to Confirmation, is scrutinized under the College of American Pathologists' Hair designation as well as ISO / IEC 17025 standards.
