For EtG, additional tests, and tests beyond 10 panels, a 150mg sample is advised. It's recommended to measure the specimen with a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be used. When referencing head hair, it specifically means scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four principal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into a lab's framework. This step verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry that the electronic chain of custody system doesn't provide.

Screening is a preliminary check for substances of abuse. Although Screening offers a cost-efficient means to eliminate drug use on most samples, a positive result must be verified for courtroom validity. Any samples with a presumptive positive in Screening demand a follow-up confirmation.

Should a specimen yield a presumptive positive at the Screening stage, additional hair is extracted from the initial sample and prepared for Extraction. This phase involves extracting drugs from hair at a considerably lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most intricate methodology to execute.

Confirmation of any positive in screening occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The whole laboratory process from Accessioning to Confirmation is evaluated according to the CAP (College of American Pathologists) Hair designation alongside accreditation to ISO / IEC 17025 standards.
