What it is, How it works

Hair encapsulates a prolonged record of alcohol and narcotics by capturing biomarkers in the growing hair strand. When extracted near the scalp, hair can reveal a detection period of up to approximately three months for alcohol and other drugs. Hair sampling is straightforward, relatively hard to tamper with, and simple to dispatch.

A sample measuring 1.5 inches of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the perfect sample size for initial screening and subsequent confirmation. For EtG, supplemental tests, and/or tests beyond 10 panels, a 150mg sample is advisable. Using a jeweler's scale for measuring the specimen is suggested. If scalp hair isn’t obtainable, an equivalent volume of body hair might be utilized. By head hair, we mean only scalp hair, whereas body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The four principal phases in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial intake of a sample into a lab’s framework. Tasks include verifying the sample’s proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and performing any further data entry not supplied by an electronic chain of custody system.

Screening comprises an initial rapid test for substance abuse. While Screening is an economical technique to exclude drug consumption in most samples, a positive result necessitates confirmation for legal admissibility. Any samples showing presumptive positivity during Screening require subsequent confirmation.

Should a sample be presumptively positive at the Screening phase, additional hair is extracted from the primary sample and prepared for Extraction. Here, drugs are withdrawn from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug testing is the toughest method to execute.

Confirmation of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation when necessary. The whole lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
