What it is, How it works

A segment of hair measuring 1.5 inches, comprising about 200 strands (roughly the dimension of a #2 pencil), taken closest to the scalp, translates into 100mg of hair, which is optimal for both screening and confirmatory purposes. For EtG, additional tests, and/or assessments beyond 10 panels, a specimen quantity of 150mg is advised. It's advisable to weigh the sample using a jeweler's scale. Should scalp hair be unavailable, a proportional amount of body hair can be utilized. References to head hair pertain exclusively to scalp hair. Body hair encompasses all alternative types (facial, axillary, etc.).

Process Overview

The four principal phases in the laboratory analysis of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial steps required for processing a sample within a laboratory's framework. This includes confirming proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not captured by an electronic chain of custody platform.

Screening consists of a preliminary rapid assessment for drugs of misuse. Although Screening provides a cost-efficient approach to excluding drug use in most samples, a positive result necessitates confirmation for court acceptance. Confirmation for any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS methods. Samples with presumptive positives are cleansed before confirmation as required. The complete laboratory sequence from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
