What it is, How it works

The use of hair testing has evolved into a significant method for spotting alcohol and drug usage. By encasing biomarkers within the fibers of hair strands, hair offers a prolonged history of these substances. When taken near the scalp, hair can present up to a three-month detection period for both alcohol and drugs. Hair is straightforward to collect, quite challenging to tamper with, and easy to dispatch.

A 1.5-inch portion containing roughly 200 hair strands (comparable to a #2 pencil's thickness) adjacent to the scalp delivers 100mg, which is the optimal amount for analysis and validation. For EtG, enhancements, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale for weighing. Should scalp hair be unavailable, a comparable quantity of body hair can be collected. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other varieties (such as facial, axillary, etc.).

Process Overview

The laboratory processes a drug test outcome through four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within the lab’s framework. This process ensures that the sample was correctly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and finalizes any supplementary data entry absent from an electronic chain of custody system.

Screening constitutes an initial rapid assessment for substance misuse. Although Screening is an economical method to discount drug consumption for the majority of samples, a positive result in Screening must be validated to be permissible in legal circumstances. Samples that show a presumptively positive result in Screening require additional confirmation.

Should a specimen show a presumptively positive result at the Screening level, further hair is extracted from the original sample and set up for Extraction. In this phase, substances are drawn from hair at a considerably reduced concentration relative to other techniques (e.g., urine or oral fluid), explaining why hair drug screening is a challenging procedure.

Verification of any positive Screening result is achieved through GC/MS, GC/MS/MS or LC/MS/MS methods. Every sample that is presumptively positive is cleaned before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is evaluated in accordance with both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
