Hair examinations have emerged as an influential method for identifying drug and alcohol consumption. Hair offers an extended history of alcohol and other substances by embedding biomarkers within the strands of the growing hair. When gathered near the scalp, hair can reveal up to roughly a 3-month detection period for alcohol and other substances. Hair collection is straightforward, relatively hard to manipulate, and simple to transport.

A 1.5-inch section of approximately 200 hair strands (about the size of a #2 pencil) nearest the scalp will yield 100mg of hair, the optimal amount for testing and verifying. For EtG, supplemental tests, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't available, a comparable quantity of body hair can be utilized. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four principal steps involved in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample into a lab's system. This step verifies that the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and fulfills any further data entry not provided by an electronic chain of custody system.

Screening entails an initial rapid review for drugs of abuse. Although Screening is a cost-efficient method to eliminate drug use on most samples, a positive screen must be confirmed to be permissible in court. Any samples that are presumptively positive in Screening necessitate additional confirmation.

If a sample proves presumptively positive during the Screening phase, more hair is extracted from the initial sample and readied for Extraction. During this stage, drugs are drawn from hair at a much reduced concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is deemed the most complex method to execute.

Verification of any positive screening outcome occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair resource and the authorization to ISO / IEC 17025 standards.

Advantages of hair drug testing:
