A 1.5-inch section of roughly 200 hair strands (equivalent to the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal amount for testing and verification. For EtG, additional tests, and/or panels exceeding 10, a sample weighing 150mg is advised. We suggest using a jeweler’s scale for measurement. If scalp hair is absent, an equivalent quantity of body hair can be gathered. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process for drug test result processing involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is logged into the lab’s database. This step ensures that the sample was sealed and delivered correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid evaluation for substances of abuse. While Screening is a budget-friendly method to eliminate substance use on the bulk of samples, a positive result must be validated for legal acceptance. Samples that show initial positive in Screening require further verification.

For samples with preliminary positive results in the Screening phase, additional hair is drawn from the original sample and prepped for Extraction. During this phase, substances are extracted from hair at a significantly reduced concentration compared to other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any positive screening outcome is undertaken through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positive results undergo washing before confirmation if necessary. The complete lab workflow from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair category and in line with ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
