For EtG, additional tests, or tests exceeding 10 panels, a 150mg sample is advised. We suggest verifying the specimen's weight on a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair can be utilized. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four core steps in processing a drug test result at the laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample within a laboratory’s system. This involves ensuring that the sample is sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any outstanding data entry not supplied by an electronic chain of custody process.

Screening is a preliminary evaluation to identify abuse substances. While cost-effective for largely dismissing drug use in most samples, a positive result from Screening necessitates confirmation for legal credibility. Samples with a presumptive positive in Screening do need further confirmation.

If a sample is tentatively positive during the Screening phase, additional hair is taken from the original sample to prepare for Extraction. In this stage, hair undergoes a process to extract drugs at far lower concentrations compared to other methods (e.g., urine or oral fluid), illustrating why hair drug screening is considered the most complex approach to execute.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptive positive are cleaned before confirmation as necessary. The full lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair category and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
