What it is, How it works

Hair analysis is now recognized as an effective means for identifying alcohol and drug usage. Hair captures a prolonged record of alcohol and drug intake by embedding biomarkers in the fibers of the hair as it grows. If taken from the scalp area, hair can reveal a detection period of up to around 3 months for drugs and alcohol. Collecting hair is easy, relatively hard to tamper with, and convenient to transport.

A 1.5-inch length of roughly 200 hair strands (roughly comparable to a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for testing and validation. For EtG, added tests, and/or tests exceeding 10 panels, a 150mg sample is suggested. We advise using a jeweler’s scale to weigh the sample. If head hair cannot be provided, an equivalent quantity of body hair might be taken. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses any other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially handled by the lab system. This step involves confirming that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not provided through an electronic chain of custody system.

Screening acts as the initial brief evaluation for drugs of abuse. Although Screening is an economical method to dismiss drug usage in the bulk of samples, a positive result necessitates confirmation for court acceptability. Samples that yield a positive preliminary result in Screening must undergo secondary validation.

Should a sample test positive during the Screening phase, more hair from the initial specimen is drawn and readied for Extraction. At this phase, drugs are retrieved from hair at much reduced concentrations than those in alternative strategies (e.g., urine or oral fluid), rendering hair drug analysis the most challenging procedure to carry out.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS or LC/MS/MS. All presupposed positive samples are cleansed before confirmation as needed. The whole laboratory procedure, from Accessioning to Confirmation, is evaluated under both CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Prolonged detection period: Hair drug exams can identify drug usage for up to 90 days, unlike urine exams which have a shorter detection period.
  • Difficult to deceive: It is extremely hard to manipulate a hair drug test, rendering the outcomes more reliable.
  • Delivers historical information: It can present a pattern of drug consumption over time, not merely recent usage.

Limitations:

  • Cannot identify recent usage: It takes nearly 5-7 days for drugs to become detectable in hair.
  • Expense: Hair drug analysis is generally more costly than other drug testing techniques.
  • Result variations: Elements like hair color and personal hair growth changes can influence the drug metabolite concentrations in the hair.

Note: Even though commonly termed "hair follicle tests", the analysis focuses on the hair strand rather than the follicle beneath the scalp
