For EtG, additional tests, and tests above 10 panels, a 150mg sample is advised. It is suggested to weigh the specimen on a jeweler’s scale. Should scalp hair be missing, an equivalent amount of body hair may serve as a substitute. The term head hair refers specifically to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test consists of four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of entering a sample into the lab’s system. This step involves affirming that the sample was properly sealed and transported, attaching a random LAN (Laboratory Accessioning Number), and inputting any additional data not covered by an electronic chain of custody.

Screening entails an initial, rapid evaluation for substances of abuse. Although Screening is economical for ruling out drug use in most samples, a positive result requires confirmation to be valid in a legal setting. Samples indicated as positive in Screening must undergo secondary confirmation.

When a sample shows a positive result in the Screening stage, additional hair is taken from the initial collection and readied for Extraction. During this phase, drugs are isolated from the hair at a much lower concentration than other methods (such as urine or oral fluid), making hair drug screening particularly challenging to execute.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation undergoes a review in line with both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.
