What it is, How it works

When taken near the scalp, hair can supply nearly a 3-month detection period for alcohol and drugs. It's easy to gather, challenging to tamper with, and straightforward to transport.

A 1.5-inch sample consisting of around 200 hair strands (comparable to a #2 pencil's width) nearest the scalp yields 100mg of hair, the optimal sample size for testing and confirmation. For EtG, add-ons, and tests exceeding 10 panels, a specimen amount of 150mg is suggested. Measuring the specimen on a jeweler's scale is recommended. If scalp hair isn't available, an equivalent volume of body hair may be used. By head hair, we mean only scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

There are four essential phases in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially logging a sample into a lab's system. This process checks that the sample was sealed and delivered correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry absent from an electronic chain of custody framework.

Screening is a preliminary check for drug misuse. Although Screening is a cost-efficient way to dismiss drug use in many samples, a positive screen must be confirmed for it to be legally valid. Any samples flagged as presumptively positive during Screening need further confirmation.

If a sample tests as presumptively positive during Screening, more hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are pulled from hair at significantly lower levels than other testing methods (e.g., urine or oral fluid), which is why hair drug testing is the most complicated approach to execute.

Confirmation of any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptively positive are washed before confirmation as necessary. The whole lab process from Accessioning to Confirmation is overseen following both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.
