A 1.5-inch sample consisting of around 200 hairs (similar to the width of a #2 pencil) nearest to the scalp yields 100mg of hair, the perfect specimen for screening and validation. For EtG, add-ons, or tests exceeding 10 panels, 150mg of hair is advised. We advocate for weighing the sample using a jeweler’s scale. In the absence of scalp hair, a similar amount of body hair may be taken. Head hair refers exclusively to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the initial entry of a sample into a laboratory’s framework. This step includes confirming that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not included by an electronic chain of custody system.

Screening offers a preliminary rapid assessment for drugs of concern. Although Screening is an economical strategy to dismiss drug usage for most samples, a screen indicating positive must be validated to be permissible in a legal setting. Any samples deemed presumptively positive during Screening require further verification.

Should a sample be considered presumptively positive during Screening, additional hair is extracted from the original sample and set for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to carry out.

Confirmation of any positive screen result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if needed. The entire laboratory procedure from Accessioning to Confirmation is inspected under CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
