It is straightforward to collect, not easily tampered with, and convenient to send.

A sample measuring 1.5 inches consisting of around 200 hair strands (about the same thickness as a standard #2 pencil) nearest the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, or those exceeding 10 panels, a specimen size of 150mg is suggested. We recommend utilizing a jeweler’s scale for measuring the specimen. Should scalp hair be unavailable, equivalent quantities of body hair can be collected. "Head hair" specifically refers to scalp hair, whereas "body hair" encompasses all other varieties (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in a laboratory is comprised of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the foundational processing of a sample into the laboratory's framework. This step ensures the sample is properly sealed and shipped, assigns it a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not already documented by an e-chain of custody.

Screening serves as an initial, swift assessment for the presence of drugs. While it is a cost-conscious method to exclude drug use in most samples, a screen test indicative of drug presence must be confirmed for legal admissibility. Any presumptively positive samples in Screening call for secondary verification.

When a sample tests as potentially positive in the Screening phase, additional hair is extracted from the primary sample for the Extraction process. Here, drugs are isolated from hair at significantly lower concentrations compared to other techniques (such as urine or oral fluid), underscoring the complexity of hair drug screening.

Positive results from Screening are verified through methods like GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation is carried out as necessary. From Accessioning to Confirmation, the entire laboratory process is scrutinized under the standards of both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
