A 1.5-inch segment of about 200 hair strands (comparable to the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal sample size for both initial and confirmatory testing. For EtG, additional analyses, and/or tests exceeding 10 panels, a 150mg sample is advised. It is recommended to use a jeweler’s scale for weighing the sample. In absence of scalp hair, an equivalent volume of body hair may be utilized. The term "head hair" specifically pertains to scalp hair, while "body hair" encompasses all other hair types, such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This phase ensures that the sample was properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data input not covered by an electronic chain of custody protocol.

Screening involves a preliminary evaluation for drugs of abuse. While Screening offers an economical method to exclude drug use in most samples, a positive screening requires confirmation to hold up in legal proceedings. Samples that appear positive during Screening need validation through a secondary confirmation process.

Should a sample test presumptively positive at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at considerably lower concentrations than in alternative methods (e.g., urine or saliva), making hair drug testing particularly complex to carry out.

Verification of any positive screening outcomes is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing presumptively positive undergo a cleansing process prior to confirmation if required. The comprehensive lab process, from Accessioning to Confirmation, is overseen according to CAP (College of American Pathologists) Hair criteria and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
