A 1.5-inch segment consisting of around 200 strands (similar in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is ideal for both initial and confirmatory tests. For EtG, additional panels, or tests exceeding 10 panels, 150mg of the sample is advised. It's recommended to weigh the sample on a jeweler’s scale. If scalp hair cannot be obtained, an equivalent volume of body hair may be used. Head hair refers exclusively to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary processing of a sample within a lab’s system. It entails confirming the sample is properly sealed and shipped, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing additional data entries beyond those provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While it’s an economical method to exclude drug use in most samples, a positive screen must be validated for legal admissibility. Presumptively positive screens necessitate further confirmation.

When a sample tests presumptively positive during Screening, more hair is extracted from the original sample to prepare for Extraction. In this phase, drugs are isolated from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing the most complex method to perform.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleaned as necessary before confirmation. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
