A 1.5-inch portion consisting of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal quantity for evaluation and confirmation. For EtG, additional analyses, and/or screenings exceeding 10 panels, a 150mg sample is advised. We advocate for using a jeweler’s scale to weigh the specimen. Should scalp hair be unattainable, an equivalent quantity of body hair can be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory examination of a drug test outcome primarily comprises four phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a specimen into a lab’s system. This step encompasses verifying that the sample was securely sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplemental data entry not supplied by an electronic chain of custody system.

Screening consists of an initial swift assessment for substances of misuse. While Screening is an economical approach to assess drug use in the majority of samples, a positive result in the screen needs confirmation for court admissibility. Samples that show a presumptive positive in Screening must undergo a further confirmation process.

If a sample is found presumptively positive during the Screening phase, additional hair is retrieved from the original sample and readied for Extraction. At this juncture, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging technique to execute.

Confirmation of any positive screen result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples presumptively positive are cleansed prior to confirmation as required. The complete lab procedure from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair designation and certified to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
