For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advised. It is recommended to use a jeweler’s scale for weighing the specimen. If scalp hair is unattainable, an equivalent amount of body hair might be gathered. References to head hair pertain solely to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial insertion of a sample into the laboratory system. This phase includes confirming the proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not captured by an electronic chain of custody system.

Screening comprises a preliminary rapid analysis for drugs of misuse. While Screening is an economical means to exclude drug consumption for the majority of samples, a positive screen necessitates confirmation to be valid in a legal context. Any samples presumptively positive in Screening require a secondary confirmation.

Should a specimen indicate a presumptive positive at the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. In Extraction, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation of positive screening findings is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing prior to confirmation, if necessary. The comprehensive laboratory procedure, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
