What it is, How it works

Collected near the scalp, hair offers up to about a 3-month detection period for alcohol and other substances. It's straightforward to collect, somewhat challenging to tamper with, and easy to send.

A sample of 1.5 inches containing roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and panels above 10, a 150mg sample is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, equivalent body hair can be used. Reference to head hair implies scalp hair only. Body hair includes all other hair types (such as facial, axillary, etc.).

Process Overview

The four primary stages involved in the lab handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing into a lab’s system. This step includes checking that the sample was sealed and dispatched properly, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening provides an initial rapid check for drug misuse. While Screening is economical for eliminating drug use in most samples, a positive screen must be confirmed to be valid in legal settings. Any samples presumed positive in Screening require secondary confirmation.

If a specimen is presumptively positive during Screening, more hair is extracted from the initial sample and prepped for Extraction. In this phase, substances are extracted from hair at much lower concentrations than other methods (such as urine or oral fluid), underscoring why hair drug screening is the most challenging method to execute.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are rinsed before confirmation as necessary. The entire lab process from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
