What it is, How it works

Hair retains a long-term record of alcohol and drug usage by embedding biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can offer up to a roughly 3-month detection period for alcohol and drugs. Hair collection is straightforward, relatively resistant to tampering, and easy to ship.

A 1.5-inch segment of approximately 200 hair strands (equivalent to a #2 pencil's size) closest to the scalp will provide 100mg of hair, the optimal sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be collected. When referring to head hair, we specifically mean scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The four core steps in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is processed into a lab’s system. This step ensures that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not available from an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. While Screening is an affordable approach to exclude drug use in most samples, a positive result must be confirmed for legal admissibility. Any samples tentatively positive in Screening must undergo a further confirmation.

If a sample is tentatively positive during the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at a notably lower concentration compared to other techniques (e.g., urine or oral fluid), which makes hair drug screening a challenging technique to perform.

Confirmation of any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Any samples presumed positive are cleansed prior to confirmation as required. The entire lab workflow from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair certification and the ISO / IEC 17025 accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
