The key advantage is that hair follicle testing provides a much extended detection window, up to 90 days, collected in a supervised process, and is nearly impossible to tamper with. However, urine testing remains most accurate for drug use detected in the past 5 days.

What it is, How it works

Viewing hair testing as a robust method for detecting drug and alcohol consumption has become prevalent. The hair stores a long-term record of alcohol and other substances by encasing biomarkers within the fibers of the hair as it grows. When cut near the scalp, hair can offer a detection span of up to approximately 3 months for alcohol and other substances. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch length of roughly 200 strands of hair (about the diameter of a #2 pencil) closest to the scalp provides 100mg of hair, which is the ideal portion for analysis and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is suggested. We advise using a jeweler’s scale for weighing the sample. If scalp hair is not available, an equivalent amount of body hair may be used. When mentioning head hair, we mean scalp hair exclusively. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory process of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial intake of a sample into a lab’s system. This process involves checking that the sample was sealed and shipped correctly, providing a random LAN (Laboratory Accessioning Number), and finishing any other data entry not supplied by an electronic chain of custody system.

Screening consists of an initial fast assessment of drug abuse. Although Screening is a cost-efficient means to eliminate drug use in the bulk of samples, a positive screen should be verified to be admissible legally. Any samples provisionally positive during Screening require an additional confirmation.

If a sample shows a presumptive positive during Screening, more hair is retrieved from the initial collection and readied for Extraction. At this phase, substances are extracted from hair at a lower concentration compared to other methods (ex. urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if needed. The complete lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
