For EtG, add-ons, or tests exceeding 10 panels, a sample weighing 150mg is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair isn’t available, an equivalent quantity of body hair may be gathered. Head hair references only scalp hair, while body hair denotes all other hair types (e.g., facial, axillary).

Process Overview

The lab processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample into a lab’s system. This task includes confirming that the sample was securely sealed and shipped, assigning it a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening performs an initial rapid examination for drugs. Although Screening is a budget-friendly way to eliminate drug usage in most samples, a positive screen requires confirmation for legal admissibility. Any presumptive positive samples during Screening need secondary confirmation.

When a sample shows a presumptive positive at the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drug concentrations are extracted from hair much lower than with other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Any positive screening outcome is verified via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessitated. The full laboratory journey from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 guidelines.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
