A 1.5-inch segment consisting of approximately 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp can yield around 100mg of hair, which is the optimal quantity for both screening and confirmation purposes. For EtG extensions and/or tests surpassing 10 panels, a quantity of 150mg of the sample is advisable. Using a jeweler’s scale is recommended to weigh the sample. If scalp hair is not available, the same volume of body hair can be used as a substitute. When discussing head hair, we specifically mean scalp hair, while body hair covers all other types (like facial, axillary, etc.).

Process Overview

The principal stages in the laboratory evaluation of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial introduction of a specimen into a laboratory’s system. This step ensures that the sample is sealed and transported correctly, assigns a unique LAN (Laboratory Accessioning Number), and addresses any further data entry absent in an electronic chain of custody system.

Screening entails a preliminary assessment for common abused substances. While Screening is a cost-effective manner to initially disregard drug usage in most samples, a positive screen mandates confirmation to be valid in a judicial setting. Any samples with presumptive positive results in Screening are subjected to further confirmation.

When a sample exhibits a presumptive positive in the Screening phase, additional hair is extracted from the original specimen for the Extraction phase. Here, substances are retrieved from hair at a significantly lower concentration compared to other methodologies (such as urine or oral fluid), thus making hair drug screening the most complex technique to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positive results undergo washing before final confirmation as necessary. The full laboratory journey from Accessioning to Confirmation is evaluated in line with both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
