In the absence of scalp hair, an equivalent amount of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair refers to all other types of hair (like facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result consists of four primary phases: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample undergoes initial processing into the lab’s system. This step confirms the sample's seal and proper shipment, assigns a random LAN (Laboratory Accessioning Number), and completes extra data entry not covered by an electronic chain of custody system.

Screening conducts a preliminary check for drug abuse. Although Screening is a cost-efficient method to dismiss drug usage in most samples, a positive screen requires confirmation to hold up in legal contexts. Samples showing presumptive positivity during Screening necessitate further confirmation.

If Screening suggests a positive result, additional hair from the original sample undergoes preparation for Extraction. During Extraction, drugs from hair are obtained at significantly lower concentrations than other techniques (like urine or oral fluid), contributing to hair drug screening's complexity.

Confirmed positive results from Screening are verified via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are washed preceding confirmation if necessary. The entire procedure, from Accessioning to Confirmation, is inspected in line with the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
